Abstract

Background: Maternal health is a critical public health issue worldwide, profoundly affecting the physical, emotional, psychological, and socioeconomic well-being of women and their families. This study aims to assess the global burden of severe pregnancy complications from 1990 to 2021, with projections extending to 2035.Methods: We assessed the global, regional, and national burden and trends of seven causes of severe pregnancy complications from 1990 to 2021, with projections extending to 2035. Our analysis focused on prevalence, incidence, disability-adjusted life years, deaths, and maternal mortality ratio (MMR).Findings: Globally, the MMR of severe pregnancy complications, except for ectopic pregnancy (EP), significantly declined from 1990 to 2021, and is projected to continue decreasing from 2022 to 2035. In contrast, the MMR for EP was increasing annually and is expected to rise further. There was a negative correlation between the MMR of these severe pregnancy complications and the Socio-demographic Index (SDI). Significant disparities existed in MMR among regions with different SDI levels。Interpretation: The reduction in the MMR of severe pregnancy complications worldwide reflects the efforts and achievements made in improving maternal health across different regions. This study highlights the need for continued focus on the early diagnosis and timely intervention of EP, particularly in low-middle SDI and low SDI areas. Furthermore, it is crucial to address the significant disparities in MMR among different countries and regions, responding effectively to the imbalances in public health development across these areas.Funding: This work was supported by grants from the Project funded by China Postdoctoral Science Foundation (2022M720295).
