Sources of High Variability of the Data that Shoulder Pain after Laparoscopic Surgery

Background: The purpose of this study was to find out the perioperative clinical factors and demographic characteristics associated with shoulder pain after laparoscopy(PLSP).
Methods: A prospective observational study was performed with 442 inpatients undergoing laparoscopic surgery for infertility.The pain visual analogue scale was used as the measuring instrument. To identify the predictors of PLSP, we performed multivariate conditional logistic regression.
Findings: PLSP was well correlated with body mass index(odds ratio (OR)=0.848). The incidence of shoulder pain and more severe shoulder pain in patients with lower BMI is significantly higher than that in patients with higher BMI, and BMI is significantly negatively correlated with PLSP. Most of the patients (97%) began to have shoulder pain after the first out of bed activity, rarely during bed rest.
Interpretation: The variation of PLSP data may be partly due to BMI and postoperative patient's position change.We should carry out more rigorous experimental design to exclude the impact of BMI and postoperative activities on PLSP data.
