Abstract
Background: We describe a case of sarcoidosis associated with nonspecific pain in the Achilles tendon area over the course of its evolution.
Case presentation: A 34-year-old woman with no medical history presented with pain unrelated to physical activity. The outward appearance of the right Achilles tendon showed thickening but was not painful on palpation. The thickening was not detected on radiography but was evident on computed tomography (CT) and magnetic resonance imaging. CT-guided biopsy showed remodeled changes, spinal cord fibrosis, and histiocytes. CD68 staining was positive for lymphocytes but negative for mycobacterial strains. Corticosteroid treatment was not prescribed, and the medication was changed yearly. This report shows images suggestive of affectation on the first finger of the other foot.
Conclusions: The clinical presentation of sarcoidosis can be complex, and differential diagnosis is challenging. Atypical clinical features also cause delays in diagnosis and proper management.
