Abstract

Background: Maternal hemorrhage remains a significant global health challenge, exhibiting notable disparities across regions and nations. It is crucial to establish a comprehensive epidemiological understanding to guide policymaking and healthcare planning.

Methods: We conducted a systematic analysis of the burden of Maternal hemorrhage at global, regional, and national levels from 1990 to 2019, utilizing data from the Global Burden of Disease (GBD) 2019 Study. Key measures, including prevalence, incidence, Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLDs), Years of Life Lost (YLLs), and Maternal Mortality Ratio (MMR), were examined and compared across different regions and time periods.

Findings: The global age-standardized prevalence of maternal hemorrhage decreased from 483.24 to 316.13 per 100,000 cases between 1990 and 2019. Similarly, the age-standardized MMR decreased from 68.83 to 34.33 per 100,000 live births. Similar trends were observed in incidence, DALYs, YLDs, and YLLs rates. The burden of Maternal hemorrhage primarily affected low and low-middle Socio-demographic Index (SDI) regions. Across all five SDI regions, there was a decrease in age-standardized prevalence, incidence, DALYs rate, and MMR. However, this progress was not evenly distributed, with East Asia experiencing the largest decrease. Both the age-standardized incidence in Australasia, and the age-standardized DALYs rate and MMR in Caribbean experienced an increase.

Interpretation: These results highlight a significant reduction in the global burden of maternal hemorrhage over the past three decades. Nonetheless, persistent regional and national disparities necessitate targeted interventions tailored to each context, to further reduce maternal mortality and morbidity.
