Clinical diagnoses of psychiatric disorders rely on clinical symptom assessments. While neuroimaging has been applied to characterize the brain in psychiatric disorders, it has not been used to diagnose major psychiatric disorders. Here, we report a neuroimaging-aided diagnostic system for major psychiatric disorders designed for clinical practice. We developed deep learning networks with attention mechanisms and applied them to a single-center neuroimaging dataset containing data from four psychiatric disorders and healthy controls (n = 2490). Cross-validations and independent validations using multicenter (n = 1346) and real-world datasets (n = 344) showed that the system used brain images to identify bipolar disorder, major depressive disorder, schizophrenia, and obsessive-compulsive disorder. The classifiers also reflected comorbidity diagnoses. Furthermore, we implemented an automated closed-loop system that provides guidance in real-world clinical settings and that further learns by accumulating data and feedback. This work advances brain research in its aim to provide objective diagnostic aid for identifying psychiatric disorders for real-world use.
